Our forms provide space for all necessary and helpful information that we require to provide the best service possible for both you and your clients.
It is helpful for us if you fax or email the referral information prior to your client arriving for their first consultation. If you have radiographs to accompany your patient please feel free to post prior to appointment (ensuring they have adequate time to be shipped) or simply send those along with your client.
It is our clinic policy that all referring veterinarian's radiographs are returned to your clinic.
